If the names are the same, go through the renewal procedures; Provide the tripartite paternity test certificate and reissue it. Second, answer specific questions 1. The newborn name is empty, and the rest of the information is complete. A parent or guardian has requested the addition of a child's name. The original birth medical certificate only fills in the parent's name, and the ID number is empty. Parents or guardians request supplementary parental identification information. Handling method: Follow the procedures for issuing and renewing the Notice. Retrieve the original hospitalization medical records with the parents' original ID documents. If the names are the same, supplement the parent's information; if the names are not the same, you need to provide a three-party paternity test certificate and reissue it. 3. The father's information (name, ID number) of the original birth medical certificate is empty, and the original hospitalization medical record is checked. The father's information is not recorded. Parents or guardians ask for more information about the father. Handling method: Follow the procedures for issuing and renewing the Notice. Examine the original parents' identity documents, the original marriage certificate (or divorce certificate) or the paternity test certificate, and supplement the father's information. If the marriage certificate is later than the hospital delivery time, a paternity test certificate must be provided. 4. The original birth medical certificate shows that the ID number of the father (or mother) is incorrect or is false information. It is the same mistake to retrieve the medical records of the original hospital delivery. The parent or guardian requested a change. Examine the tripartite paternity test certificate and modify the identity information according to the "Renewal Application Form". 5. After the medical institution fully issued the "Birth Medical Certificate", the mother of the newborn claimed that the father did not bear the responsibility of raising children due to unmarried children, and requested that the father's information be cancelled.
